[ARCHIVED] H1N1 Vaccination for Middle and High School Students

Students will receive vaccinations within the next 2 weeks.
Vaccination is free, however, appropriate forms must be completed in full and must be received by the middle and high school clinics by December 7th 2009.

Parents must:
1. Review the 2009 H1N1 Influenza Vaccine statement (VIS Sheet) before completing any paperwork
2. Download the 2009-2010 Vaccine Administration Record
3. Download 2009 H1N1 Flu Vaccine Consent Form

(Forms are also available in the main office of each middle and high school building and the town of Andover website )

Both forms must be completed in full and signed by a parent/guardian and returned to the school clinic by Monday December 7th 2009.
Students with Incomplete forms will not be vaccinated.
Vaccine will be given by injection only.

Following vaccination, each child will be provided with an immunization record card. This is your record of immunization and must be shared with your child’s primary care provider.

The Health Department will be planning additional H1N1 clinics as vaccine become available. Check the Town website for further information.
